Check the bearing on the other end of the countershaft where the chain rides. If it were bad the movement could force the idler into the mainshaft gear.

Thank you copemech . The bearing is ok , no movement . I think , i have to open the cases .

can you turn the shafts freely? With chain removed.

If something is bent you should see it when you rotate it, could even be crank pin slipped. Take plug out and spin crankshaft too.

Hi, I may be guessing here but the wear in the teeth is irregular like somthing is not running true or there is play somewhere.

Could it be the backplate on the clutch, the bit where the rivets are, is bent or warped causing the gear to not run true?

The backplate on the clutch is not bent or warped . Thats my realy problem , everything looks ok , and my engineer say that the problem is the quality of the gears .
